This sponsorship form relates to an event, which is trying to raise money for my treatment in America. I have a type of cancer called Neuroblastoma. Its quite rare and for the last 15 months I have been fighting it through chemotherapy, radiotherapy and surgery. I have got through some really hard times and am ok at the moment but my family are worried about my future and want to help me beat the odds.
